Wage for a CCC:
- Starting wage between $41.95/hour - 48.95/hour
- Wage Includes $0.85/hour supplemental pay through 9.30.2026

Gladwin Pines Nursing & Rehabilitation Center is accepting applications for a Clinical Care Coordinator RN. This position is integral in optimizing our residents' ability to meet their fullest potential. The CCC is responsible for the MDS assessment and management of the residents through the review of medical records, quality indicator reports, care plans and physical assessments. Prepares, provides and documents medication and treatment administration whenever necessary.
