We are seeking a skilled Social Worker (LBSW) to join our team of dedicated professionals.
Key Responsibilities:
- Provide psychosocial support to patients and their families based on comprehensive assessments of their needs and goals.
- Collaborate with the interdisciplinary team to develop and implement plans of care that address the unique needs of each patient and family.
To be successful in this role, you will require a Bachelor's Degree in Social Work from a CSWE-accredited school and at least one year of experience working as a Social Worker in a healthcare or hospice setting.
