Job Summary
We're looking for a Social Worker (LBSW) to join our team.
Responsibilities
The Social Worker (BSW) is a key member of the interdisciplinary team, providing psychosocial support to patients and their families. This includes assessing needs, identifying goals, interventions, and services based on an established plan of care.
Requirements
- Bachelor's Degree in Social Work from a CSWE accredited School
- One year experience as a Social Worker in a healthcare and/or hospice setting
- Ability to apply knowledge of special needs of hospice patients and families
