Role Overview
We are seeking a skilled Social Worker (LBSW) to join our team. The ideal candidate will be a member of our interdisciplinary team providing psychosocial support to patients and their families based on thorough assessments.
Key Responsibilities:
- Conduct thorough psychosocial assessments to identify patient/family needs, goals, interventions, and services required according to the established plan of care.
- Utilize professional training and personal judgment to monitor the psychosocial assessment process.
Requirements:
- Bachelor's Degree in Social Work from a CSWE-accredited school.
- One year of experience as a Social Worker in a healthcare or hospice setting is required.
- Ability to apply knowledge of the special needs of hospice patients and families.
