Position - Case Manager
Regional Health Systems is Northwest Indiana’s largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children—both uninsured and insured—integrated care through our community mental health center and federally qualified health center. Regional Health Systems is a member of the Regional Care Group.
Position Description:
We are seeking a Case Manager to help clients overcome challenges, access vital resources and create paths towards better health and stability. The ideal candidate will play a critical role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of behavioral health, child welfare, and crisis management to effectively assist clients in navigating their challenges and achieving their goals.
Pay is based on years of experience:
$17.51 - $22.76
Essential Functions:
- Complete comprehensive assessments to understand client needs and strengths
- Develop personalized service plans and track progress over time
- Support clients in navigating housing, healthcare, child welfare, and work force systems
- Collaborate with healthcare teams, social workers and community partners
- Provide crisis intervention and use motivational interviewing techniques
- Keep clear and timely documentation of client progress and service coordination
Requirements:
- Bachelor’s Degree in Psychology, Sociology or related field
- 1 year of experience in case management, social services or behavioral health
- Valid driver’s license and reliable transportation
