Position - Case Manager
Regional Care Group is seeking a dedicated Case Manager to join our team in Merrillville. This vital role involves coordinating comprehensive care plans to support individuals across our diverse community programs. Based in Merrillville, you will play a key part in advancing our mission to deliver accessible, holistic healthcare and social services.
Responsibilities:
- Develop individualized care plans tailored to clients needs
- Complete comprehensive assessments to understand client needs and strengths
- Provide crisis intervention and support during critical situations.
- Advocate for clients to access appropriate resources such as behavioral health services.
- Maintain accurate documentation of client interactions and progress.
- Collaborate with other departments to ensure continuity of care.
- Support program development initiatives aimed at improving service delivery.
Requirements:
- Bachelor's Degree in Psychology, Sociology or related field
- 1 year of experience in case management, social services or behavioral health
- Valid Driver's License

Pay: $17.93 - $23.31 per hour
