Description:
Gateway Rehab Center (GRC) has an opening for an LPN or RN Extended Care Nurse in Center Township, PA. In this role, you will provide assessment, nursing diagnosis/treatment planning, and direct nursing care to GRC patients. This position receives supervision and instruction from the Executive Regional Director of Nursing.
Responsibilities
- Conducts patient triage, assessments, reassessments, medical interventions, phlebotomy & POC testing as needed.
- Complete audits for the extended care programs as required.
- Interacts with the physician through coordination and the provision of assistance via in person, telephone, med wand or other platforms.
- Collaborates with pharmacy on medication issues.
- Coordinates with all GRC departments as appropriate and care coordination with outside agencies as needed.
- Attends GRC mandatory in-services and trainings.
Knowledge, Skills, and Abilities
- Excellent oral and written communication skills
- Strong computer skills.
- Pennsylvania or Compact RN/LPN unrestricted license.
- One year nursing experience, addiction medicine and med surg nursing experience preferred.
- Active CPR certified required.

- Working conditions include potential high-risk exposure to blood borne pathogens and low-risk exposure to respiratory infectious agents.
- Moderate physical demands.
- Significant mental demands.
- Travel between various locations.
- Operation of various office and safety equipment.
- Use of various healthcare equipment and measurement devices.
- Requires mandatory staffing coverage when necessary to provide a safe treatment environment for GRC residents.
- Must remain awake and alert while on duty.
