What You'll DoSupport residents and families throughout their stay
Assist with admissions, discharges, and care planning
Coordinate care conferences and communicate with interdisciplinary teams
Connect residents and families with community resources
Advocate for residents' rights and emotional well-being
Maintain accurate documentation and ensure regulatory compliance
Long-term care or skilled nursing experience preferred
Previous social services or healthcare experience preferred
Strong communication and organizational skills
Compassionate, professional, and resident-centered approach
LSW a plus, but not required
