The Social Services Assistant assists with functions of the Social Services Department under the direction of the Director of Social Services.
Essential Job Duties:
1. Assist Director of Social Services with completion of Social Histories, Admission and Quarterly Assessments and necessary documentation.
2. Maintain daily communication with Director of Social Services about residents’ status.
3. Assists with coordination of the dental, vision and hearing programs; and creates necessary documentation.
4. One on one visits with residents per Plan of Care and tracking of visits
5. Assist with referrals to community agencies as needed (i.e. Level of Care, Representative Payee, and Guardianships).
6. Assist Director of Social Services in coordinating discharges and discharge planning.
7. Act as resident advocate and assist in maintaining resident rights.
8. Assist in scheduling Family Care Conferences.
9. Perform other duties and assume other responsibilities as the Director of Social Services may assign.
Qualifications
1. Must hold a high school diploma or equivalent.
2. Prefer one year experience working with the elderly in a medical setting.
3. Ability to communicate in a clear, concise manner.
4. Ability to deal with and appropriately handle confidential information.
