Position Summary
The Social Services Assistant supports the Social Services Department under the direction of the Social Worker. This role assists with resident assessments, documentation, discharge planning, community referrals, and coordination of services to help ensure residents receive compassionate, person-centered care.
Essential Responsibilities
- Assist the Social Worker with Social Histories, Admission Assessments, Discharge documentation, insurance reviews, and Medicare-related forms.
- Maintain daily communication with the Social Worker regarding resident status and needs.
- Coordinate dental, vision, hearing, podiatry, and optometry services for residents.
- Complete and maintain required documentation related to resident services.
- Assist with referrals to community agencies, including Level of Care, Representative Payee, and Guardianship services.
- Assist with discharge planning and resident transitions.
- Advocate for resident rights and quality of life.
- Schedule Family Care Conferences.
- Coordinate resident voter registration and absentee ballots.
- Assist with monitoring and completion of HENS and PASRR screenings.
- Perform other duties as assigned.
Qualifications
- High school diploma or GED required.
- One year of experience working with older adults or in a healthcare setting preferred.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information professionally.
- Proficient with Microsoft Office and basic computer applications.
- Strong organizational skills with attention to detail.
- Ability to work collaboratively with residents, families, and the interdisciplinary care team.
