Job Title: Nurse Care Manager
Location: Oklahoma City, OK
Schedule: Monday - Friday, 8:00 AM – 5:00 PM
Pay: $34.67 hourly

Qualifications:
- Valid Oklahoma RN license or recognized by the Enhanced Nurse Licensure Compact (eNLC)
- Minimum of 2 years of professional nursing experience
- Strong clinical knowledge, care coordination, and case management skills
- Excellent communication and stakeholder collaboration abilities
- Ability to analyze complex situations and develop effective care plans
- Experience working with Medicaid/SoonerCare populations is preferred
- Proficiency in MS Office Suite (Word, Excel, Access, PowerPoint) is a plus
- Bilingual skills are advantageous
- Physical ability to perform office tasks, including extended sitting and computer use
- Must be able to work in a standard office environment with accessible facilities
Responsibilities and Duties:
- Coordinate and facilitate access to medical care for members with complex healthcare needs.
- Conduct clinical reviews and document care management activities using available systems.
- Complete assessments and level of care determinations utilizing standardized tools.
- Collaborate with internal and external stakeholders to ensure comprehensive care delivery.
- Prepare reports, cost-effectiveness statements, and other documentation as required.
- Participate in case staffing meetings and contribute to care planning.
- Interface with members, families, and providers to resolve issues and advocate for their needs.
- Act as a resource for OHCA personnel regarding members’ complex healthcare needs.
- Monitor program adherence to state and federal guidelines through QA processes.
- Represent the organization in public forums, seminars, and training sessions.
- Evaluate healthcare service appropriateness and make recommendations for care plans.
- Perform other duties as assigned to support program goals.
