Our team is made up of dedicated professionals who support one another like family—and we’re currently looking for an experienced Weekend Hospice Nurse Supervisor (RN) to lead our weekend care team with heart and purpose.
The Weekend Nurse Supervisor plays a vital role in the clinical and administrative operations of our hospice admissions and visit teams during the weekend. This role oversees after-hours and weekend patient care, supports field staff, and ensures continuity of care by triaging calls, making visits, and directing hospice services across Friday evening through Monday morning.

Responsibilities- Provide clinical supervision for weekend and on-call hospice staff.
- Triage patient calls, perform skilled assessments, and make or assign visits as needed.
- Complete hospice admissions and follow-up visits with professionalism and compassion.
- Initiate and revise plans of care in response to changing patient needs.
- Maintain clear and timely communication with interdisciplinary team members.
- Ensure smooth coordination of care during weekend transitions (Friday PM and Monday AM).
- Accurately document clinical care and communicate patient status changes.
- Offer guidance and mentorship to weekend field staff, fostering a collaborative team environment.
- Current RN license in the state of Pennsylvania (required).
- 2 years of hospice or palliative care experience (required).
- 1 year of leadership or supervisory experience (preferred).
- Strong assessment and triage skills, with the ability to manage urgent needs.
- Compassionate communication with patients, families, and staff.
- Familiarity with hospice regulations and Medicare/Medicaid requirements.
- Valid driver's license and reliable, insured transportation.
