Job Description:
Are you a Licensed Practical Nurse (LPN) with a heart for hospice and a passion for ensuring patients receive coordinated, compassionate care? Gallagher Hospice is seeking a dedicated Hospice Coordinator (LPN) to oversee care coordination, weekend scheduling, and patient services for our growing hospice team. This position offers 3x12 shifts (Friday, Saturday, and Sunday)—perfect for nurses who want a consistent weekend schedule with full-time hours.
As a key member of our care team, you’ll play a vital role in supporting patients and families through their end-of-life journey. You’ll be responsible for coordinating weekend patient care, managing schedules for field staff, and ensuring continuity and quality of care—all while being a compassionate resource for patients, families, and staff.
Key Responsibilities:
- Coordinate patient care and staffing during weekend hours to ensure timely, compassionate hospice services.
- Prepare and maintain weekend schedules for field staff, ensuring equitable assignments and continuity of care.
- Serve as the point of contact for patient, family, and referral source needs during the weekend shift.
- Communicate and collaborate with interdisciplinary team members, including RNs, social workers, and chaplains.
- Support accurate documentation and timely updates to clinical records.
- Assist with insurance verification, authorizations, and related care logistics as needed.
- Perform direct care visits as needed, including covering LPN visits to support patient needs and maintain service continuity.
Qualifications:
- Licensed Practical Nurse (LPN) with current state licensure.
- Minimum of 1 year hospice or healthcare experiencestrongly preferred.
- Experience in scheduling, coordination, or clinical operations is a plus.
- Strong communication, organizational, and multitasking skills.
- Compassionate, patient-centered mindset with a team-first attitude.
Shift:
Fridays, Saturdays, Sundays – 12-hour shifts (3x12)
