Position Summary
The LPN provides care to clients in Home Health, at their place of residence, and within ADHS. The LPN reports to and works under the supervision of the ADHS Center Director. The nurse renders all care as ordered by the client’s medical provider, acts as a member of the multi-disciplinary team and liaison between healthcare professionals, completes nursing assessments and reassessments, maintains records to ensure agency and regulatory compliance, reports any changes in orders and/or client condition to the supervising RN and client’s provider, and provides health education to staff, clients, and their families/caregivers.

Requirements
Education/Experience/Credentials/Knowledge/Understanding
- 18 years of age or older
- Have and maintain AZ Licensed Practical Nurse License
- Have and maintain Fingerprint Clearance Card
- Have and maintain CPR certification
- Have and maintain Food Handlers Card
- Have and maintain AZ Driver’s License and acceptable MVR
- Have and maintain reliable transportation and current vehicle insurance
- Meet all state and federal immunization and communicable disease clearance requirements
- This position is classified as Safety Sensitive
