Role Description
The Home Health Care Manager (Attendant Care Manager) is a full-time, on-site position based in Bowling Green, KY. As an Attendant Care Manager, you will play a vital role in enhancing the quality of life for participants of the Medicaid Home and Community Based waiver program. The Attendant Care Manager is responsible for the implementation and operational management of our in-home non-skilled care services and programs.
What you’ll do
- Review KHAT, POC & sign MAP, 116, admission docs w/ case managers
- Plans, manages, and directs the operation of activities for the in-home care services.
- Participates in the recruitment, onboarding, educating and supervising all of the Attendant Care staff.
- Oversees the assessment and review of the participants' care environment and appropriateness of services to assure the participants' safety and individualized choice opportunities is maintained according to federal and state guidelines and company policy.
- Develops individualized participants' home care service plan.
- Facilitates the investigation and review of any grievance or complaints regarding care or services provided for our participants
- Investigate, f/u & address all in-home incident reports
- PAs into Therap (KY state EMR); denials/liabilities
- Collaborative partnership w/ ADHC Admin/Nurse
Qualifications
- Required: RN or LPN with a minimum of 2 years previous management experience in home health services working with seniors and/or intellectual and developmental disabilities (IDD).
- Required: Active license in Kentucky
- Strong understanding of home health care regulatory compliance, Medicaid services, and care coordination standards.
- Proficiency in electronic health records (EHR) systems and healthcare documentation.
- Strong communication skills to effectively interact with participants, families, Attendant Care staff, and community partners.
- Proficient with Microsoft Office Suite
- Leadership and team management skills, including the ability to supervise a diverse caregiving staff and multidisciplinary teams.
- Experience in planning, implementing, and monitoring care plans to ensure high-quality service delivery.
- Excellent communication and interpersonal skills for engaging with clients, families, staff, and strategic partners.
- Administrative abilities such as scheduling, documentation, and training coordination.
- Ability to work effectively in a culturally diverse environment, with sensitivity to various languages and cultures.
- Current valid driver’s license and reliable transportation.
