Role Description
This is a full-time on-site role for a Case Management Specialist located in Louisville, KY. Working under the supervision of the Case Manager Supervisor, the Case Manager Specialist will coordinate and communicate with Medicaid home care participants, families, and Case Managers to ensure participants’ needs are met. Assist in the facilitation of Case Management referrals.
Key tasks include conducting assessments, managing client cases, collaborating with healthcare and social service providers, ensuring compliance with applicable regulations, and maintain accurate and confidential records and support compliance through internal audits.. The role requires maintaining accurate documentation and serving as a key point of contact for clients and their families throughout their care journey.
Qualifications
- LPN with a minimum of 2 years of case management experience, preferred.
- Valid Kentucky LPN License required
- Knowledge of community resources, entitlement programs, and service coordination
- Excellent Communication and interpersonal skills for client and family interactions
- Ability to work collaboratively with healthcare providers and community resources
- Knowledge of Medicaid-focused care and HCBS is highly desirable
- Prior experience working with seniors, individuals with disabilities, or veterans is a plus
- Proficient in Microsoft Office Suite
