What are the responsibilities and job description for the MDS / RAI Coordinator position at Frasier 2025?
- Lead and coordinate the Resident Assessment Instrument (RAI) process across disciplines
- Complete and submit Minimum Data Set (MDS) assessments accurately and on time
- Collaborate with nursing, therapy, and other team members to gather and validate information
- Ensure compliance with CMS and state regulatory requirements
- Monitor assessment schedules and follow up to keep everything on track
- Support care planning by ensuring assessments reflect each resident’s needs
- Serve as a resource to the team on MDS and RAI requirements
- Participate in on-call rotation responsibilities
- Current, active RN license in Colorado (in good standing)
- At least 3 years of experience as a Registered Nurse, including 1 year with MDS completion
- Solid understanding of Medicare, Medicaid, and state/federal regulations related to MDS
- Working knowledge of ICD-10 coding
- Experience with electronic health records (PCC experience a plus)
- Strong computer skills and comfort with clinical documentation systems
- Clear communication skills and the ability to work effectively with residents, families, and team members

Compensation
- Pay Range: $33.30 - $42.45 per hour
