Role Overview
The RN MDS Coordinator is responsible for coordinating and assisting with the accurate and timely completion of interdisciplinary MDS Assessments, Care Plans, and CCAs in line with CMS regulations and facility standards.

The RN MDS Coordinator is responsible for coordinating and assisting with the accurate and timely completion of interdisciplinary MDS Assessments, Care Plans, and CCAs in line with CMS regulations and facility standards.
Key Responsibilities
- Coordinate the submission of MDS Assessments, Care Plans, and CCAs
- Report patient condition changes identified during assessments
- Educate staff on updates to CMS RAI Manual and regulations
- Review and validate assessment reports and ensure proper documentation in CMS QIES system
- Monitor CMS Reports for assessment timeliness and develop process improvements
- Participate in interdisciplinary meetings and review care plans
- Perform staff nursing duties as needed
- Provide excellent patient service and support safety protocols
- Associate’s or bachelor’s degree in nursing from an accredited institution
- Active RN license in good standing
- Minimum of two years’ nursing experience, preferably in geriatric care
- CPR certification (obtained upon hire or during orientation)
- Advanced knowledge of post-acute care practices, regulations, and procedures
- Ability to make independent decisions and demonstrate professionalism
- Strong communication, confidentiality, and teamwork skills
- Maintain compliance with company policies and code of conduct
- Exhibit excellent judgment, reasoning, and interpersonal skills
- Ability to perform physical activities such as lifting and moving patients
