Frasier is hiring an Admissions Nurse (RN) to own clinical review and coordination across our continuum of care, supporting appropriate placement and smooth transitions for residents and families.
This role sits at the center of the admissions process—evaluating referrals, coordinating next steps, and ensuring every admission is handled clearly, consistently, and on time.

Pay Range: $76,200 - $97,100 yearly-based on experience and qualifications.

WHAT YOU’LL OWN
Clinical Review & Admission Determination
- Review referrals, hospital records, and clinical documentation to determine appropriateness for admission
- Assess acuity, anticipated care needs, and alignment with available services and licensure
- Apply sound clinical judgment to identify risks, concerns, and placement suitability
Admissions Process Management
- Manage admissions from initial referral through move‑in
- Coordinate insurance verification, authorizations, and required documentation
- Track progress, follow up on outstanding items, and ensure timelines are met
Communication & Coordination
- Serve as a primary point of contact for families, referral partners, and internal teams
- Clearly explain services, expectations, timelines, and next steps
- Ensure accurate and timely handoff of information at admission
Assessments & On‑Call Support
- Complete admission‑related nursing assessments as required
- Participate in the clinical on‑call rotation, providing after‑hours guidance as scheduled
WHAT YOU KNOW AND HAVE EXPERIENCED
- Current, unrestricted Registered Nurse (RN) license in the state of Colorado
- Graduation from an accredited nursing program
- Experience in skilled nursing, hospital case management, admissions, or care transitions
- Strong clinical assessment and decision‑making skills related to admission appropriateness
- Familiarity with Medicare, managed care, and insurance authorization processes
- Clear, professional communication and strong organizational skills
- Ability to manage multiple priorities with accuracy, consistency, and follow‑through
