Frasier is seeking an Admissions Coordinator to support residents and families through admissions across our continuum of care. This role plays a critical part in creating a welcoming, well-coordinated admissions experience grounded in respect, accountability, and professionalism.

Pay Range: $36.65-$46.70 per hour based on experience and qualifications.

WHAT YOU’LL OWN
Guide Residents and Families
- Serve as the primary point of contact from initial inquiry through admission
- Provide clear, compassionate guidance about services, levels of care, timelines, and next steps
- Support families through complex and emotional care transitions with professionalism and empathy
Manage the Admissions Process
- Coordinate the full admissions workflow across Skilled Nursing, Assisted Living, Memory Support, and Adult Day
- Collect, review, and track required documentation, including medical records and insurance authorizations
- Ensure admissions processes meet regulatory, organizational, and payer requirements
Review Referrals & Coordinate Placement
- Partner with clinical leadership to assess appropriateness for admission
- Consider clinical, psychosocial, and operational factors when coordinating placements
- Support smooth transitions by aligning expectations and coordinating move-in logistics
Collaborate Across Teams
- Work closely with nursing leadership, social services, marketing, therapy partners, and external providers
- Ensure clear handoffs and continuity of care at the time of admission
Support Systems & On-Call Coverage
- Maintain organized workflows across multiple concurrent admissions
- Identify opportunities to improve efficiency and admissions outcomes
- Participate in the Health & Supportive Services on-call rotation as needed
WHAT YOU KNOW AND HAVE EXPERIENCED
- Bachelor’s degree in Social Work, Human Services, Healthcare Administration, or related field (required)
- Master’s degree in Social Work (MSW) or related discipline (preferred)
- Minimum of three years’ experience in hospital case management, skilled nursing, or related healthcare setting
- Experience with admissions, discharge planning, or care transitions (preferred)
- Knowledge of post-acute care environments and insurance authorization processes
- Strong organizational, communication, and follow-through skills
