About the Role
We are seeking a detail-oriented and dependable Clinical Intake Coordinator to support patients served by Fortis Health–affiliated agencies. In this role, you will manage incoming referrals, coordinate admissions, and work closely with patients, referral sources, and clinical teams to ensure a smooth and timely start of care.
Responsibilities
- Receive and process incoming referrals for home health and hospice services
- Enter and maintain accurate patient demographic, insurance, and clinical information
- Verify insurance coverage and obtain necessary authorizations
- Review referral documentation for completeness and compliance
- Coordinate with clinical teams to ensure patient appropriateness and safe admissions
- Communicate with referral sources, patients, and internal teams to facilitate timely start of care
- Track and follow up on pending referrals to ensure timely decisions
- Escalate concerns related to clinical appropriateness, staffing, or service capabilities
- Maintain HIPAA compliance and patient confidentiality
- Support ongoing quality improvement and team collaboration

- Comfortable working with EMR systems and learning new technology
Physical Requirements:
- Primarily desk-based role with prolonged computer use
- Frequent communication via phone and electronic systems
- Ability to remain focused on detailed tasks for extended periods

Qualifications:- Active LPN or RN license (required)
- 2–3 years of experience in home health, hospice, intake, or care coordination (preferred)
- Experience with insurance verification and authorizations (required)
- Knowledge of home health/hospice regulations and OASIS (preferred)
