Essential job duties for this position include the following items. Other duties must be performed as assigned or required.
▪ Understands Home Care and non-medical services provided
▪ Provides and coordinates services in accordance with the plan of care.
▪ Makes the initial evaluation visit and regularly reevaluates the client’s needs.
▪ Initiates the plan of care and necessary revisions.
▪ Initiates appropriate preventive and rehabilitative non-medical procedures.
▪ Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.
▪ Informs personnel (caregivers) of changes in the condition and needs of the client.
▪ Understands and adheres to established policies and procedures.
▪ Implements the nursing care plan for each client.
▪ Observes, records, and documents signs and symptoms and reports to the physician, Administrator or Care Coordinator reactions to non-medical treatments, including drugs and changes in the Client's physical or emotional condition.
▪ Communicates and counsels the client and family/significant others regarding their service needs and other related problems of the client at home.
▪ Participates in quality assurance audits, as assigned.
▪ Attends staff meetings, client care conferences and in-services as scheduled.
▪ Submits any changes in schedule to Administrator/Care Coordinator/Nursing Supervisor on a daily basis.
▪ Supervises and teaches other personnel; ie Caregivers, PCAs and HHAs.
▪ Assists the client in learning appropriate self-care techniques.
▪ Assists with other activities, as assigned.
EDUCATION/EXPERIENCE
▪ Graduate of a state approved school of nursing and currently licensed in the state(s) in which practicing.
▪ Minimum of one (1) years’ experience in nursing, non-medical home care experience preferred.
▪ Proficient skills in Microsoft Office and scheduling software preferred
▪ Must possess and demonstrate excellent organizational, communication, interpersonal and leadership skills as well as a positive and professional image
CERTIFICATION, LICENSURE, & REGISTRATION
▪ Maintain current state Registered Nurse (RN) licensure
