First Choice Home Health & Hospice is seeking a compassionate, organized, and empathetic Bereavement Coordinator to join our hospice team. This individual will provide emotional support and bereavement services to families before and after the loss of a loved one, helping them navigate the grieving process with dignity, compassion, and hope.
Responsibilities
- Develop and implement individualized bereavement care plans for hospice families.
- Provide grief support through phone calls, visits, mailings, support groups, and community resources.
- Conduct bereavement risk assessments and identify individuals who may benefit from additional counseling or referrals.
- Recruit, train, and supervise bereavement volunteers.
- Collaborate with the interdisciplinary hospice team to ensure continuity of care for patients and families.
- Maintain regular communication with community resources, clergy, counselors, and support organizations.
- Assist in educating staff and volunteers on grief, loss, and bereavement care.
- Ensure all bereavement services meet federal, state, and accreditation standards.
Qualifications
- Experience in hospice, grief counseling, social services, or a related healthcare setting preferred.
- Excellent communication and interpersonal skills.
- Strong organizational and documentation abilities.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Comfortable providing compassionate support during emotionally difficult situations.
- Valid driver's license, reliable transportation, and current auto insurance.
