Role: Intake Coordinator (RN or Therapist) responsible for facilitating patient admissions, conducting initial assessments, and developing treatment plans in a mental health and addiction treatment setting.
Key Duties: Perform direct admissions from hospitals, assess patients, consult with patients, families, and medical staff, and provide counseling on care needs and referrals.
Qualifications: Master's degree in social work or mental health counseling OR BSN required, along with an unrestricted Utah license. Minimum of 2 years' post-graduate experience in psychiatric or substance abuse treatment, preferably in inpatient or partial hospitalization settings. Familiarity with managed health care, medical terminology, case management, discharge planning, and utilization review is preferred.
