RN Case Manager- $5,000 SIGN ON BONUS

Job Title: RN Case Manager

Reports to: Clinical Manager

Pay: Up to $125,000 per year, plus a $5,000 sign-on bonus.

Hours: Monday-Friday 8am-5pm. Expectation to work 1-2 weekend days per quarter.

We have immediate openings for Home Health Registered Nurses (RNs) Case Managers to provide skilled nursing services to patients at home. This role initiates the nursing plan of care with preventative and rehabilitative nursing procedures.

We have multiple territories throughout our 60 mile coverage area in the Willamette Valley. Your assigned territory will be discussed based on your preferred location that is closest to your home. Each RN sees approx. 5-7 patients a day with an ongoing caseload of approximately 25-30 patients.

Responsibilities:

• Make initial evaluations and regularly reevaluates the patient's nursing needs including updating the plan of care.
• Provides skilled nursing services to include but not limited to: catheter care, wound care, disease management teaching, and medication management.
• Works closely with the patient/family/caregivers and other members of the interdisciplinary team
• Supervise, delegate, teach and train aspects of patient care to other members of the interdisciplinary team
• Ability to meet the productivity expectations of the agency.

Job Requirements:

• Current & valid license to practice in Oregon
• Current & valid Driver's license
• Current & valid car insurance
• CPR certified
• Dependable car
• Excellent critical thinking skills, time management, & ability to be flexible
• 1 year of acute experience preferred but not required
• Home health or SNF experience preferred but not required
