Position: Home Health Certified Nursing Assistant (CNA)
Job Duties:
- Provide one-on-one personal care to patients in their homes following a patient-specific care plan developed by the RN Case Manager.
- Duties include bathing and personal hygiene, ambulation and transfer assistance, vital signs monitoring, and other activities of daily living (ADL).
- Collaborate closely with patients, families, caregivers, and other members of the interdisciplinary team.
- Complete point-of-care documentation accurately and timely.
- Meet the agency's productivity expectations.
Qualifications:
- Current Oregon CNA license.
- Minimum of 1 year of full-time direct patient care experience.
- Prior experience with Electronic Medical Records (EMR) is preferred.
- Ability to maintain composure in stressful and challenging situations.
- Current and valid Oregon driver's license and car insurance.
- CPR certification.
- Reliable vehicle.
- Ability to function effectively as a member of the interdisciplinary team, communicating with the team at least weekly.
