Primary Care Physician (MD/DO) – Mission-Driven Community Health

As a Primary Care Physician, you will be a trusted partner in your patients’ health journeys. You will deliver comprehensive primary care, champion preventive health efforts, and help create a welcoming environment where individuals feel heard and valued. Your clinical expertise and leadership will have a direct, lasting impact on community well-being.

Key Responsibilities:

• Provide patient-centered care, including diagnosis, treatment, and management of acute and chronic conditions.
• Conduct physical exams and coordinate necessary diagnostic testing.
• Educate patients on preventive care, treatment plans, and healthier lifestyles.
• Collaborate with a multidisciplinary team to deliver coordinated, high-quality care.
• Support Patient-Centered Medical Home (PCMH) initiatives to improve outcomes and access.
• Mentor and support clinical staff and medical students.
• Participate in on-call coverage in collaboration with partner physicians.

Qualifications & Skills:

• Education: MD or DO degree from an accredited institution.
• Licensure/Certifications: Active Indiana medical license, Board Certified (participating in MOC), Current CPR certification, and eligible for hospital credentialing.
• Technical Skills: Proficiency in managing acute/chronic conditions, physical exams, and PCMH framework initiatives.
• Soft Skills: Deep compassion, empathy, strong communication, collaborative mindset, and a passion for serving diverse populations.
