Position Summary:
- Seeking an experienced Director of Social Work to lead social work services in a long-term care/healthcare setting.
- This position is responsible for assessing residents’ psychosocial needs, developing and overseeing care plans, advocating for residents, supervising social work staff, and coordinating services with families, healthcare professionals, government agencies, and community organizations.
Key Responsibilities:
- Direct and oversee medically necessary social work services and manage assigned caseloads.
- Supervise, guide, and evaluate social work staff and case management activities.
- Conduct biopsychosocial assessments and participate in interdisciplinary care planning.
- Support residents individually and in groups to maximize psychosocial functioning and independence.
- Develop interventions for behavioral concerns, including crisis intervention and behavior modification.
- Serve as a resident advocate regarding psychosocial, medical, financial, legal, and residents’ rights matters.
- Assist with abuse investigations and prevention initiatives.
- Oversee utilization review and assist with level-of-care determinations and discharge planning.
- Coordinate with Medicaid, Medicare, Social Security, local social services, disability services, legal aid, and other community agencies.
- Mediate concerns and conflicts involving residents, families, and staff.
- Support admissions activities, including facility tours and nursing facility admissions.
- Assist with Medicaid reviews, resident financial resources, and patient fund account monitoring.
- Prepare required reports, evaluations, documentation, quality assurance assessments, and monthly administrative reports.
- Develop and coordinate resident and family engagement programs.
- Monitor bed utilization and Medicaid requirements.
- Maintain and update social work policies and procedures in accordance with applicable regulations.
- Participate in surveys, inspections, staff meetings, and in-service training.
- Maintain compliance with HIPAA, residents’ rights, and applicable federal and state regulations.
Qualifications:
- Master’s degree in Social Work (MSW) or equivalent.
- Minimum 1 year of related experience in a nursing home, assisted living facility, hospital, or social services setting.
- Geriatric mental health experience preferred.
- Strong knowledge of social work practices, healthcare regulations, Medicare, Medicaid, and related benefits programs.
- Experience with long-term care or assisted living environments preferred.
- Strong supervisory, communication, organization, documentation, and problem-solving skills.
- Ability to work independently, exercise sound judgment, and manage multiple priorities.
- Proficiency with standard computer applications and electronic documentation systems.
- Strong understanding of confidentiality and HIPAA requirements.
Physical & Work Requirements:
- Ability to sit, stand, walk, bend, stoop, kneel, reach, push/pull, and perform other routine physical activities throughout the workday.
- Ability to lift up to 25 pounds unassisted.
- Ability to communicate effectively and understand written and verbal instructions.
- Ability to work effectively with residents, families, staff, and individuals who may be distressed or emotionally upset.
- May require occasional evenings, weekends, extended hours, or emergency response responsibilities.
