RESPONSIBILITIES:
- Assess the psychosocial status of patients/families/caregivers related to the patient's illness and environment
- Carry out social evaluations and plan interventions based on evaluation findings
- Counsel patient/family/caregivers as needed in relationship to stress and other identified coping difficulties
- Maintain clinical record on all patients referred to social work
- Provide information and referral services for organization patient/family/caregivers regarding practical and environmental needs
- Maintain collaborative relationships with organization personnel to support patient care
- Maintain and develop contacts with public and private agencies as resources for patient and personnel
- Participate in discharge planning when needed
REQUIREMENTS:
- License in Social Work preferred
- Experience in Long Term Care preferred
