Home Care RN Job Description
A Home Care Registered Nurse provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. These nurses manage pain and other symptoms, provide emotional support, and advise families or care takers on treatment while a medical professional is not present.
Key Responsibilities:
- Assess and chart observations of the patient’s condition at each visit.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Administer Physician-prescribed medication.
- Dress or redress wounds and assess the healing progress.
- Provide education to patients and families on proper home health care procedures and strategies.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
- Keep the patient comfortable through palliative care.
- Recommend tools or devices that may improve the quality of life for the patient.
Qualifications and Requirements:
- RN license in state of assignment. Minimum 3yrs experience
- BLS required
- Prior experience in hospice, palliative care, or home health is strongly preferred.
- HHC, EPIC EMR, and lab draws.
- Strong assessment and clinical decision-making skills; excellent communication; emotional intelligence; ability to work independently in diverse home settings.
Work Environment:
- Primarily home-based care, including private residences, assisted living facilities, or nursing homes.
- Travel between patient locations is required; mileage reimbursement is often provided.
- Flexible scheduling, but may include evenings, weekends, and on-call rotations depending on agency needs.
- Shifts & Scheduling: 8-4:30 M-F, plus weekend.
- Weekend Requirements: We ask they work every 4th weekend, regular day shift 8 - 4:30 Saturday and Sunday and have scheduled days off thru the week for time off.
Physical and Emotional Demands:
- Ability to lift, transfer, or assist patients safely.
- Comfortable navigating various home environments and conditions.
- High level of emotional resilience to support patients and families through end-of-life care.
- Capacity to work independently with minimal supervision while managing emotionally charged situations.
