Director of Social Services

We are seeking a motivated Director of Social Services to join our team in Swansea, MA. This leadership position plays a critical role in overseeing social services and ensuring that residents receive the emotional, psychosocial, and supportive care they need to thrive.

The ideal candidate will have strong leadership and communication skills, experience in skilled nursing or long-term care, and a genuine commitment to supporting residents and their families.

• Lead, plan, develop, and oversee comprehensive social service programs and initiatives.

• Supervise, mentor, and support social services staff while promoting professional development.

• Ensure social services are delivered in accordance with federal and state regulations, facility policies, and professional standards.

• Advocate for residents and ensure their psychosocial, emotional, and social needs are identified and appropriately addressed.

• Develop and maintain positive relationships with residents, families, community organizations, healthcare providers, and other stakeholders.

• Collaborate with nursing, administration, rehabilitation, activities, and other departments to provide coordinated, person-centered care.

• Participate in interdisciplinary care planning and resident care conferences as appropriate.

• Assist residents and families with appropriate community resources, referrals, and support services.

• Support behavioral health and psychosocial needs through appropriate interventions and resources.

• Monitor and evaluate social service programs to identify opportunities for improvement.

• Maintain accurate and timely documentation in accordance with regulatory and facility requirements.

• Assist with managing social services resources and program budgets effectively.

• Promote resident rights, dignity, independence, and quality of life.

• Help foster a positive, respectful, compassionate, and supportive environment for residents, families, and staff.

• Participate in quality improvement initiatives and facility programs as required.

• Bachelor’s degree in Social Work, Social Services, or a related field preferred.

• Current and valid Massachusetts Social Worker license required.

• Previous experience in a skilled nursing facility (SNF), nursing home, long-term care, or healthcare setting required.

• Strong knowledge of social services, psychosocial care, behavioral health, and long-term care regulations.

• Proven leadership and staff management abilities.

• Excellent communication, interpersonal, organizational, and problem-solving skills.

• Strong ability to advocate for residents and their families.

• Compassionate, professional, and resident-focused approach.

• Ability to work effectively as part of an interdisciplinary healthcare team.

• Strong documentation and record-keeping skills.

• Must be able to pass required background and employment checks.
