We are a private, not-for-profit organization that coordinates public health grants for a 26-county region in central PA, seeking a Community Health Worker (CHW). This is a full-time hybrid position located within our Camp Hill location, balanced with opportunities for work-from-home days.
The Community Health Worker (CHW) will provide housing support services and other case management duties as assigned to clients with lower incomes in their communities. CHW will work collaboratively with all applicable internal and external stakeholders' staff and partners to identify needs and connect clients to housing, healthcare and social services.
A High School Diploma or GED (General Education Diploma) is required. Prior CHW experience or related experience is a plus. CHW Certification will be required; training will be provided upon hire. The successful candidate will also possess strong verbal and written communication skills, time management, and prioritization; computer literacy including Microsoft 365; and the ability to work with cross-disciplinary teams of diverse individuals, including those with lived experience. Reliable transportation and a valid PA driver's license are required for travel within the specified area.

- FBI Criminal Record Check, from fingerprinting
