What You'll Do
As an LPN, you will work closely with providers, nurses, and other members of the care team to provide comprehensive clinical support, including:
- Assess and monitor patients' conditions and conduct health risk assessments.
- Prepare patients for examinations, treatments, and diagnostic procedures.
- Administer medications, treatments, and immunizations as ordered by providers.
- Reconcile medications and maintain accurate medication lists in the electronic health record.
- Provide education to patients regarding medications, treatments, health maintenance, disease prevention, and healthy lifestyle choices.
- Obtain specimens and perform appropriate in-house laboratory testing.
- Coordinate and track laboratory, radiology, and other diagnostic results and follow up with patients as directed.
- Assist with identifying and addressing gaps in care and support patient panel management.
- Coordinate and track referrals, including referrals to community resources.
- Maintain accurate, timely, and complete documentation in the electronic health record.
- Participate in daily team huddles and pre-visit planning.
- Maintain clinical supplies and ensure appropriate inventory for the care team.
- Maintain quality control of in-house testing equipment and monitor/document refrigerator temperatures.
- Sterilize equipment according to established protocols.
- Follow all infection prevention, safety, and biohazardous waste procedures.
- Provide professional, compassionate, and respectful service to patients, families, visitors, and coworkers.
- Support the overall goals of the clinical team and perform other duties as assigned.
