Iii. Primary Responsibilities
- Function as the Care Manager within the integrated primary care and behavioral health team.
- In coordination with the BHC and primary care provider, enroll eligible and interested patients into the Circle of
- As often as is clinically appropriate, face-to-face visits and telephone calls with Circle of Care patients and other
- Establish and maintain a patient registry to monitor clinical outcomes as well as key processes of care.
- Arrange for inpatient, partial hospitalization, intensive outpatient programs as needed. Assist with discharge
- Coordinate information flow between the PCP, BH providers, CHW, and other FCHC staff within the Circle of Care
- Attend care team meetings and document clinical information/decision-making in the patient registry.
- Identify barriers to treatment and possible solutions, including coordinating/offering transportation when needed.
- Establish and maintain cooperative referral relationships with community agencies, organizations, and

- Facilitate referrals to psychiatry as needed.
- Participate in staff in-service programs related to social services and/or behavioral health.
- For LCSWs only: Provide clinical supervision for licensure to LMSWs.

- Participates in Center work team as needed.
- Participates in Center staff problem solving groups.
- Performs other duties as assigned.

Vi. Qualifications
- MSW required. LMSW, LCSW preferred.
- Minimum of three (3) years experience working in the field of social work or behavioral health care is required.
- Work experience or strong desire to work in urban community health setting preferred.

- Satisfactory maintenance of continuing education standards as established by the Division of Professional
