Physical Therapist

Role: Licensed Physical Therapist responsible for delivering personalized, one-on-one care to older adults in private residences and senior living communities.

Key Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
• Document patient progress accurately within the EMR system, ensuring compliance with regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to extend access and flexibility when appropriate.
  
  

Qualifications & Skills

• Valid PT license or eligibility in applicable state(s).
• Degree from an accredited Physical Therapy program.
• Proficiency in computers and EMR systems.
• Current CPR certification.
  
  

Physical Requirements: Ability to perform moderate physical efforts, assist with patient transfers, reposition patients, and conduct therapeutic activities, including driving between locations and wearing PPE when necessary.
