We are seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary team. The PT will evaluate, develop, and implement individualized treatment plans for residents recovering from illness, surgery, injury, or mobility impairments. The goal is to help patients regain mobility, improve functional independence, and enhance overall quality of life. The PT will work closely with Physical Therapist Assistants (PTAs) and other rehabilitation team members to ensure residents receive the highest standard of care.

- Assess residents’ physical abilities and limitations, and assist in developing individualized treatment plans.
- Implement therapy programs, including therapeutic exercises, gait training, functional activities, and modalities (e.g., ultrasound, electrical stimulation, heat/cold therapy) under PT supervision.
- Supervise, collaborate with, and support PTAs in executing treatment plans.
- Monitor and document resident progress, adjusting interventions and reporting changes to the supervising PT.
- Educate residents, families, and caregivers on exercises, mobility techniques, safety, and injury prevention.
- Prepare and maintain treatment areas and equipment in a safe, sanitary manner.
- Participate in interdisciplinary care planning and discharge planning.
- Ensure compliance with federal, state, and facility rehabilitation regulations and professional standards.
- Perform additional duties as assigned by the Director of Rehabilitation.
- Location: Fairport Rehabilitation and Nursing Center – Fairport, NY Pay Range: $50.00 – $55.00 per hour Schedule: Per diem; must have weekend availability
- Doctor of Physical Therapy (DPT) or Master’s degree from an accredited program.
- Valid New York State PT license.
- Experience in skilled nursing facilities (SNF), rehabilitation centers, or geriatric care preferred.
- Strong knowledge of rehabilitation techniques, patient safety, and clinical assessment.
- Excellent communication, teamwork, and documentation skills.
- Ability to motivate and encourage patients, providing patient-centered care.
- Knowledge of geriatric rehabilitation and ability to work effectively in a multidisciplinary team.
- Familiarity with electronic medical record (EMR) systems a plus.
