NATURE OF WORK
Fairfield Memorial Hospital is seeking a compassionate, proactive, and patient-focused Population Health Nurse Educator to join our Horizon Healthcare Clinic team. The Population Health Nurse Educator serves as a vital link between patients, providers, caregivers, and community resources, helping individuals navigate the healthcare system and take an active role in managing their health. This position focuses on chronic disease management, patient education, care transitions, population health initiatives, and quality improvement efforts designed to enhance patient outcomes and reduce barriers to care. The ideal candidate combines strong clinical expertise with exceptional communication skills, a passion for patient advocacy, and a commitment to delivering high-quality, patient-centered care in a collaborative outpatient setting.

KEY RESPONSIBILITIES
  • Coordinate care and facilitate communication among patients, providers, caregivers, and community resources.
  • Provide education to patients, families, and community members regarding chronic disease management, including diabetes, hypertension, and other chronic conditions.
  • Conduct Social Determinants of Health (SDOH) assessments and support Chronic Care Management (CCM) initiatives.
  • Coordinate discharge planning and transition-of-care activities to improve continuity of care.
  • Support patients with insurance assistance programs, medication assistance programs, and advance care planning.
  • Manage and coordinate the Target BP Initiative.
  • Oversee Low-Dose CT program management and registry reporting requirements.
  • Assist with Continuous Glucose Monitoring (CGM) setup, patient education, troubleshooting, and downloads.
  • Perform quality audits and support Merit-Based Incentive Payment System (MIPS) initiatives and regulatory reporting.
  • Promote patient engagement through education and utilization of the patient portal.
  • Collaborate with interdisciplinary teams to identify barriers to care and implement solutions.
  • Participate in departmental quality improvement and compliance efforts.
  • Float to satellite clinic locations and provide clinical support as needed.
  • Educate patients regarding available community resources and support services.
  • Serve as a resource for support groups and community educational programs.
  • Perform other duties as assigned.

COMPENSATION: $24.00-$36.00 per hour

SCHEDULE: Monday- Friday 8:00 am to 5:00 pm

EDUCATION/QUALIFICATIONS
  • Required: Graduate of an accredited School of Nursing; Current, active, unrestricted Registered Nurse (RN) license in the State of Illinois.
  • Preferred: Bachelor of Science in Nursing (BSN); Chronic disease management or patient education certifications; Experience with diabetic education and population health initiatives.

EXPERIENCE
  • Required: Minimum of 2–3 years of clinical nursing experience.
  • Preferred: Previous experience in community health, primary care, care coordination, case management, population health, or chronic disease management; Experience working with diabetic patients, insulin pumps, and Continuous Glucose Monitoring (CGM) systems.

SKILLS
  • Strong understanding of chronic disease management and patient education principles.
  • Excellent communication and interpersonal skills.
  • Ability to effectively educate diverse patient populations and caregivers.
  • Knowledge of care coordination, discharge planning, and care transition processes.
  • Proficiency with Electronic Health Records (EHR) and quality reporting systems.
  • Strong organizational and time-management skills.
  • Ability to collaborate effectively with providers, patients, families, and community resources.
  • Commitment to patient-centered care and continuous quality improvement.
  • Ability to prioritize multiple responsibilities in a fast-paced clinical environment.
