Job Title: Scheduler / Medical Records Coordinator (Home Health Skilled Experience Required)
Job Summary:
We are currently seeking an experienced Scheduler / Medical Records Coordinator with a strong background in Home Health Skilled services. The ideal candidate must be familiar with home health workflows, clinician scheduling, and regulatory documentation requirements. This role is essential in ensuring efficient coordination of patient care and accurate medical records management in compliance with all applicable guidelines.
Key Responsibilities:
Scheduling:
- Schedule and coordinate visits for Skilled Nursing, Physical Therapy (PT), Occupational Therapy (OT), and Home Health Aide (HHA) services.
- Ensure visits are scheduled according to physician orders, frequency, and Plan of Care.
- Manage and adjust clinician schedules efficiently based on geography and patient needs.
- Communicate with patients, caregivers, and field staff to confirm and update appointments.
- Handle last-minute changes, missed visits, and rescheduling in a timely manner.
Medical Records:
- Maintain and manage patient records within EMR systems (WellSky/Kinnser or similar).
- Review documentation for accuracy, completeness, and compliance prior to submission.
- Track physician orders, OASIS documentation, and visit notes.
- Follow up with clinicians to ensure timely completion of documentation.
- Ensure compliance with HIPAA, Medicare, and home health regulations.
- Assist with audits, QA processes, and survey readiness.
Qualifications (REQUIRED):
- Minimum 1–2 years of experience in a Home Health Skilled Agency (REQUIRED).
- Strong knowledge of home health scheduling and documentation processes.
- Experience with EMR systems (WellSky/Kinnser preferred).
- Understanding of OASIS, Plan of Care, and physician orders.
- Excellent organizational and multitasking skills.
- Strong communication skills (English required; Spanish is a plus).
