Key Responsibilities
- Conduct interviews with residents and families to develop personalized care plans
- Collaborate with families to promote inclusivity, independence, and well-being
- Assist with discharge planning and provide solutions for resident needs, including financial resources
- Work closely with interdisciplinary teams to ensure holistic, resident-centered care
- Advocate for residents by addressing social and emotional concerns and maintaining clear communication
- Minimum of 2 years of experience as a Social Worker in a Skilled Nursing Facility (SNF) required
- Strong communication and interpersonal skills
- Compassionate, organized, and resident-focused
- Experience with Sigma EMR system preferred
