Key Responsibilities
- Develop, implement, and evaluate comprehensive social service programs for residents
- Coordinate admissions, transfers, and discharges efficiently and effectively
- Complete social service assessments, MDS documentation, care plans, and evaluations
- Participate in resident assessments, discharge planning, and interdisciplinary care plan meetings
- Conduct interviews with residents and family members to identify psychosocial needs and develop appropriate interventions
- Assist residents and families with accessing community resources, transportation services, and supportive programs
- Educate residents and families regarding Medicare, Medicaid, financial assistance programs, and available benefits
- Maintain current knowledge of federal, state, and local regulations governing long-term care facilities
- Collaborate closely with nursing staff, physicians, rehabilitation teams, and other interdisciplinary team members
- Ensure accurate and timely documentation within PointClickCare (PCC) and other required systems
- Maintain detailed progress notes and resident records
- Participate in quality assurance initiatives, facility surveys, and regulatory inspections
- Serve on committees including Quality Assurance, Infection Control, and Policy Advisory
- Ensure resident wishes regarding MOLST/POLST and end-of-life care are documented and respected
- Bachelor's Degree in Social Work, Human Services, or a related field required
- Associate's Degree may be considered with significant relevant experience
- Minimum of two (2) years of social services experience in a Skilled Nursing Facility (SNF) required
- Strong background in discharge planning and care coordination
- Prior long-term care or nursing home experience required
- Experience with PointClickCare (PCC) or similar electronic medical record systems preferred
- Excellent communication, organizational, and interpersonal skills
- Strong problem-solving abilities and attention to detail
- Ability to work collaboratively in a fast-paced, resident-focused environment
