Key Responsibilities
- Assist the Occupational Therapist with resident evaluations and the implementation of individualized treatment plans
- Support residents in performing activities of daily living (ADLs) and therapeutic exercises
- Help residents work toward rehabilitation and functional independence goals
- Monitor, document, and report resident progress and responses to treatment
- Collaborate with the interdisciplinary care team to ensure the highest quality outcomes
- Maintain resident confidentiality and comply with all regulatory and facility standards
- Current New York State COTA License
- Experience using Net Health software preferred
- Prior long-term care or skilled nursing experience strongly preferred
- Strong communication, teamwork, and organizational skills
