Key Responsibilities
- Direct and oversee all social services programs and services within the facility
- Coordinate discharge planning, financial assistance, and community resource referrals
- Ensure psychosocial and behavioral health needs are addressed through individualized care planning
- Advocate for resident rights and provide education regarding advance directives and available services
- Participate in resident and family meetings to support communication and care planning
- Maintain knowledge of federal, state, and local regulations governing long-term care facilities
- Develop and update policies and procedures related to social services operations
- Collaborate with interdisciplinary teams to improve resident outcomes and quality of life
- Support regulatory surveys, audits, and compliance initiatives
- Establish and maintain relationships with community agencies and healthcare resources
- Provide support and guidance to residents, families, and staff regarding social service concerns
- Maintain accurate documentation in accordance with facility and regulatory requirements
- Bachelor's degree required; Social Work or related field preferred
- Minimum of two (2) years of social work experience in a healthcare or long-term care setting
- Prior supervisory or leadership experience preferred
- Strong knowledge of skilled nursing facility regulations and resident rights
- Excellent communication, leadership, and organizational skills
- Ability to work effectively with residents, families, staff, and community partners
- Compassionate, professional, and resident-focused approach to care
