Job Title: RN MDS Coordinator

Facility: Palm Beach Nursing Center

Location: Lake Worth, Florida

Shift: Full-Time | Day Shift, 9:00 AM - 5:00 PM

Pay Rate: $42-$46 per hour (based on experience)

Palm Beach Nursing Center is seeking a skilled and detail-oriented RN MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process. The ideal candidate will be well-versed in PDPM and Medicare documentation, ensuring compliance with state and federal regulations while supporting exceptional resident care.

Key Responsibilities:

• Accurately complete and coordinate all MDS assessments in compliance with regulations.
• Develop and maintain individualized care plans based on assessment outcomes.
• Collaborate with interdisciplinary teams to ensure continuity and quality of care.
• Monitor PDPM requirements and ensure documentation supports Medicare claims.
• Review and analyze QI/QM reports, implementing corrective actions when necessary.
• Participate in surveys, audits, and inspections.
• Submit assessments electronically and maintain organized records and schedules.
• Educate staff on MDS/PDPM updates and best practices.
  
  
  

Requirements:

• Active RN license in the state of Florida.
• Proven experience with PDPM and Medicare documentation.
• Minimum of 2 years’ experience in a skilled nursing facility preferred.
• Strong knowledge of MDS 3.0, care planning, and RAI processes.
• Excellent communication, leadership, and organizational skills.
