Key Responsibilities:
- Administer medications and treatments in accordance with physician orders and facility protocols.
- Create and implement care plans that address residents’ individual needs.
- Provide skilled nursing care including wound care, vital sign monitoring, and reporting changes in condition.
- Maintain accurate and timely documentation.
- Ensure residents' rights and privacy are respected in accordance with HIPAA and facility policies.
- Current, valid LPN license in the State of New Jersey.
- Skilled Nursing Facility experience preferred but not required.
