Key Responsibilities
- Conduct comprehensive evaluations and design individualized therapy plans tailored to residents’ needs and goals.
- Deliver evidence-based interventions that promote independence in daily living.
- Monitor progress, document outcomes, and adjust treatment plans as needed.
- Collaborate with a skilled multidisciplinary team to ensure residents receive the highest quality care.
- Support residents recovering from illness, injury, or disability with personalized rehabilitation services.
- Maintain accurate documentation and safeguard resident confidentiality.
- Create a positive, respectful, and compassionate environment for residents and families.
- Graduate of an accredited Occupational Therapy program.
- Active New York State Occupational Therapist license.
- Prior experience in long-term care or skilled nursing facilities preferred, but not required.
