Key Responsibilities
- Promote Excellence in Care: Oversee and monitor resident care quality, ensuring all services meet the highest standards.
- Comprehensive Resident Assessments: Conduct detailed evaluations and interviews to identify residents’ needs and guide individualized care planning.
- Strategic Care Planning: Develop and maintain thorough, goal-oriented care plans focused on improving resident outcomes and quality of life.
- Collaborative Leadership: Coordinate and facilitate interdisciplinary team meetings with caregivers, therapists, and families to align on care objectives.
- Community Partnership: Build and maintain relationships with community specialists and healthcare partners to optimize resident care.
- Team Integration: Participate actively in nursing management meetings to foster open communication and cohesive teamwork.
- Education & Training: Lead in-service training sessions on MDS, RAPS, and Triggers to promote staff growth and compliance.
- Regulatory Compliance: Ensure strict adherence to the MDS and care planning schedule, particularly for non-Medicare residents.
- Licensure: Active LPN or RN license in the State of Florida.
- Experience: Minimum of 2 years of experience working in a Skilled Nursing Facility (SNF).
- Expertise: In-depth understanding of MDS, Medicare regulations, and the care-planning process.
