Key Responsibilities
- Coordinate and complete all MDS assessments in accordance with CMS and state regulations
- Ensure accurate implementation of RAPS, triggers, and care planning processes
- Maintain and update MDS and care plan policies and procedures
- Monitor and manage assessment schedules to ensure timely submissions
- Track Quality Indicators (QI) and Quality Measures (QM) and implement corrective actions as needed
- Collaborate with the interdisciplinary team, including nursing, therapy, social services, and dietary
- Assist with discharge planning by coordinating care plan components
- Participate in facility surveys, audits, and inspections
- Support quality assurance and performance improvement initiatives
- Facilitate interdisciplinary care plan meetings
- Communicate effectively with residents, families, and external providers
- Evaluate residents’ functional abilities to support therapy and activity planning
- Active Florida RN or LPN license (required)
- Minimum of 2 years of nursing home experience preferred
- Strong knowledge of MDS, PPS, QI/QM, and care planning processes
- Excellent organizational, communication, and teamwork skills
- High level of professionalism, integrity, and accountability
