Briarcliff Manor Center for Rehabilitation and Nursing Care is seeking a detail-oriented and experienced MDS Coordinator (RN) to join our team in a hybrid capacity. This role plays a critical part in ensuring clinical compliance, optimizing reimbursement, and supporting high-quality, resident-centered care.
Key Responsibilities
- Coordinate and complete all MDS assessments (admissions, quarterly, annual, and significant change) accurately and on time
- Ensure compliance with CMS guidelines, state regulations, RAPs, and triggers
- Collaborate with the interdisciplinary team to develop and maintain comprehensive care plans
- Monitor and analyze Quality Measures (QM), Quality Indicators (QI), and CASPER reports
- Participate in audits, surveys, and regulatory inspections
- Support discharge planning and care plan meetings
- Maintain accurate documentation and ensure HIPAA compliance
- Active, unencumbered RN license in New York State
- Bachelor’s Degree in Nursing required
- Minimum 2 years of supervisory experience in a hospital or skilled nursing setting
- At least 6 months of experience in rehabilitative/restorative nursing
- Strong background in skilled nursing/nursing home environments
- In-depth knowledge of MDS, CMS regulations, and care planning processes
