Director of Social Services

Facility: Rehabilitation Center of Winter Park

Location: Winter Park, Florida

Schedule: Full-Time | Monday–Friday | 8:00 AM – 4:30 PM

Salary: $65,000 – $85,000 per year

We are seeking a dedicated and empathetic Director of Social Services to lead and oversee programs that support the emotional, social, and behavioral well-being of our residents.

Key Responsibilities

• Lead and oversee all social services programs under the direction of the Administrator.
• Conduct comprehensive resident assessments to identify psychosocial needs and available support resources.
• Coordinate community resources, referrals, and support services for residents and families.
• Collaborate with the Interdisciplinary Team (IDT) to align social services with individualized care plans.
• Assist residents and families with discharge planning, financial assistance, and community placement resources.
• Facilitate access to behavioral health services and psychosocial support programs.
• Participate in resident council and family meetings while advocating for resident rights and concerns.
• Educate residents and families regarding resident rights, advance directives, living wills, and related resources.
• Maintain compliance with all state and federal regulations and recommend policy updates as needed.
• Provide translation and interpretation assistance for residents with limited English proficiency when applicable.
• Maintain accurate, timely, and professional documentation in accordance with facility standards.
• Participate in surveys, audits, compliance initiatives, and quality improvement programs.
  
  
  

Qualifications

• Bachelor’s degree in Social Work or related human services field such as Sociology, Gerontology, Psychology, or a related discipline.
• Minimum of one (1) year of experience in a skilled nursing facility or healthcare-related setting preferred.
• Strong leadership, communication, and independent decision-making skills.
• Compassionate and resourceful professional with the ability to work effectively with residents of varying cognitive levels.
• Knowledge of Microsoft Windows, Excel, PowerPoint, Spreadsheets, PCC, and Outlook.
• Ability to collaborate effectively with residents, families, healthcare professionals, and community agencies.
• Good standing with applicable state and/or national licensing or professional organizations.
