We are seeking a dedicated and empathetic Director of Social Services to lead and oversee programs that support the emotional, social, and behavioral well-being of our residents.
Key Responsibilities
- Lead and oversee all social services programs under the direction of the Administrator.
- Conduct comprehensive resident assessments to identify psychosocial needs and available support resources.
- Coordinate community resources, referrals, and support services for residents and families.
- Collaborate with the Interdisciplinary Team (IDT) to align social services with individualized care plans.
- Assist residents and families with discharge planning, financial assistance, and community placement resources.
- Facilitate access to behavioral health services and psychosocial support programs.
- Participate in resident council and family meetings while advocating for resident rights and concerns.
- Educate residents and families regarding resident rights, advance directives, living wills, and related resources.
- Maintain compliance with all state and federal regulations and recommend policy updates as needed.
- Provide translation and interpretation assistance for residents with limited English proficiency when applicable.
- Maintain accurate, timely, and professional documentation in accordance with facility standards.
- Participate in surveys, audits, compliance initiatives, and quality improvement programs.
- Bachelor’s degree in Social Work or related human services field such as Sociology, Gerontology, Psychology, or a related discipline.
- Minimum of one (1) year of experience in a skilled nursing facility or healthcare-related setting preferred.
- Strong leadership, communication, and independent decision-making skills.
- Compassionate and resourceful professional with the ability to work effectively with residents of varying cognitive levels.
- Knowledge of Microsoft Windows, Excel, PowerPoint, Spreadsheets, PCC, and Outlook.
- Ability to collaborate effectively with residents, families, healthcare professionals, and community agencies.
- Good standing with applicable state and/or national licensing or professional organizations.
