Key Responsibilities
- Oversee the admissions process from referral through admission
- Serve as the primary liaison between hospitals, physicians, case managers, families, and interdisciplinary teams
- Review clinical documentation for admission appropriateness and authorization requirements
- Coordinate with nursing, therapy, social services, and finance departments to ensure smooth transitions
- Manage bed availability, census development, and admissions tracking
- Build and maintain strong referral source relationships to support occupancy growth
- Educate referral partners on facility services, specialty programs, and outcomes
- Support patients and families throughout the admission and transition process
- Collaborate with leadership to achieve occupancy and revenue goals
- Minimum 2 years of experience in admissions, intake, case management, or healthcare business development
- Prior experience in a Skilled Nursing Facility (SNF) or post-acute care setting strongly preferred
- Previous Admissions Director or Admissions Coordinator experience required
- Strong understanding of Medicare, Medicaid, Managed Care, and authorization processes
- Familiarity with healthcare regulations related to admissions and transfers
- Experience working with interdisciplinary clinical teams
- Proficiency with EMR and referral management systems including PointClickCare, CarePort, NaviHealth, and AllScripts
